1) How to process Philippine Passport?

2) What are the documentary requirements? 

  • New/Lost Passport 
*Birth Certificate authenticated by NSO with O.R. with form 102 certified true copy issued by local civil registrar’s office with O.R.
*Marriage Certificate authenticated by NSO with O.R.– If applicant is Married and Female, with form 13 certified true copy and issued by local civil registrar’s office with O.R.
*Valid National Bureau of Investigation (NBI) Clearance
*Identification Card at least (2) two – valid or not valid
*CFO certificate – Commission on Filipino Overseas – If married to FOREIGN NATIONAL.
*Supporting Documents
-Original Baptismal Certificate/
-Voters Certification with O.R. issued by Comelec within Negros Oriental/
-School Records (Form 137, Records of Grades, College Transcript) 
  • Either of the (3) three – if  late registrant 
*Original Baptismal Certificate
*Voters Certification with O.R. issued by Comelec within Negros Oriental

  • ADDITIONAL REQUIREMENTS FOR LOST PASSPORT 
*Police Report stating passport information of the latest passport issued by DFA (if lost valid passport only)
*Affidavit of Loss stating passport information of the latest passport issued by DFA 
  • Renewal 
*Old Passport and photo copy; 
*Two supporting documents (Original Baptismal Certificate/Voters Certification with O.R. issued by COMELEC  within Negros Oriental/School Records (Form 137, Records of Grades, College Transcript))
*2 pieces Identification Card (valid or not)
*Valid Seaman’s Book – If seaman 
  • Amendment 
*Original passport
*Any amendment supporting document- must be authenticated from NSO (e.g. marriage certificate, birth certificate, death certificate, correction of name entry order and certificate of finality etc .) 
  • Requirements for MINOR (18 years old and below) 
*Birth Certificate with O.R. authenticated by NSO with form 102 certified true copy issued by local civil registrar’s office with O.R.
*Marriage Certificate with O.R authenticated by NSO with form 13 certified true copy and issued by local civil registrar’s office with O.R. if applicable 
  • IF NOT TRAVELLING WITH PARENTS (additional requirements) 
*Affidavit of support and consent with Special Power of Attorney duly notarized from the nearest Philippine embassy of the parents location
*DSWD clearance – Department of Social Welfare and Development
*Any proof of identity of the traveling companion

5) Who needs a DSWD clearance Certificate and What are the Requirements in Securing DSWD Clearance and its application form?

  • *A minor traveling alone to a foreign country ;
     
  • *A minor traveling to a foreign country accompanied by a person other than his or her parents.The requirements are as follows and the clearance is valid for one (1) year from the date of issue.  A two (2) years valid clearance can also be requested.
 
*Duly accomplished application form;
*Photocopy of the birth certificate or passport of the minor;
*A written consent of both parents or the solo parent or the legal guardian permitting the child to travel alone or with someone else to a foreign country;
*Photocopy of passport of traveling companion, for accompanied minors. 
  • *As appropriate,
 
*a photocopy of the marriage certificate of the minors parents or;
*a photocopy of the certificate of legal guardianship of the minor or;
*a photocopy of the solo parent identification card from the municipal social welfare and development office or Tallaq or Fasakh Certification from the Shariah court or any Muslim Barangay or religious leader;in the case of an illegitimate minor, a certificate of no marriage from the local civil registrar;in the case of a deceased parent, a photocopy of the death certificate;

6) Where can we obtain a passport application form? 

  • Please click below for the sample application form, please print it using red ink, laser printer, 8 x 14 inches size white bond paper on the first and second page, it must be printed back to back in one bondpaper only, and the third page must be printed using black ink, laser printer and printed on 8 x 14 inches white bond paper, please complete it and bring them to our office yet, our office will not guarantee acceptability of the said application form since printing quality may differ and therefore not acceptable during processing at the Department of Foreign Affairs.

8 ) What will happen after submission of my supporting documents? 

  • Submitted documents are pre assess by the documentation officer in charge yet final assessment of the supporting documents is to be done by the Department of Foreign Affairs processor in charge.

9) How much is the cost of the passport Processing?

10) What happen next? 

  • Upon receipt of  payment, you will be schedule to comply your personal appearance at the Department of Foreign Affairs.  Upon completion of the personal appearance, you will get your original passport at our office at the specified time given by the documentation in charge.

11) My Philippine Passport has expired or about to expire and I am not in the Philippines, can I send my passport to the Philippines and have it renewed? 

  • No. We can only renew Philippine Passport provided it has a latest arrival stamp that corresponds to latest departure.  These are one of the guidelines known to us with regards to processing of Philippine Passport.  You may proceed to the nearest Philippine Embassy for this purpose.
